A virtual Community of Practice session will take place on 16th February 2023 for Care Coordinators and Social Prescribers in Primary Care who are supporting cancer care/processes.

These sessions are specifically designed to support Care Coordinators and Socal Prescribers within Primary Care, who are working to support cancer processes or patients living with a cancer diagnosis.
